Making palliative care support and services available in rural and remote communities

About the Program (  Factsheet)

Palliative Care is the specialised, multidisciplinary, and holistic care of someone suffering from an advanced, progressive terminal illness. The Rural Communities Palliative Care project specifically focuses on coordinating the case management of patients requiring palliative care and supporting their families and friends. It also supports peer training and education of local Multidisciplinary Teams and develops and supports Link Nurses in each local rural community.
